I've had both done and in my experience the nipples were slightly more painful and a bigger hassle because it took much longer to heal, whereas my hood was healed within 48hours or so. Talking with the piercer, the nipple logically seems to be more painful because they have to pierce more tissue and nerves than with the hood.But in the end it all goes up to the person's threashold of pain.
