Unintentional Riser Release

I am aware of two cases within the past year similar to this one:

Basically, the problem is one riser releasing on opening for no apparent reason. When reviewed, the gear appears to be correctly manufactured and assembled.

Has anyone seen a malfunction like this? Does anyone know what can cause it?

This is an extremely worrisome problem, as it is creating high speed malfunctions in a no-reserve environment.

Can anyone help me out here?

Maybe I'm missing something, or simply don't know enough about BASE jumping, but I've always wondered why base rigs were even manufactured with cutaway risers. I mean, if you have an off-heading opening & slam into the wall of a cliff and find yourself stranded on a ledge somewhere, I can see how it would be helpful, but otherwise.... what's the point?

BASE rigs are manufactured with three rings because there have been several BASE fatalities (offhand, I'd estimate five or so) related to drowning. Jumpers landed in water and could not cut away canopies. It's also handy to be able to cut away in a "jump and run" situation, or when hung up in a tree/lamppost/etc.

I have only seen this when the teflon cable is too short or the teflon is rigged through the grommet before the loop....... but those count as incorrect assembly........ so possibly dont answer your Q......

I'm trying to picture this but am having a hard time. Do you mean the grommet that is attached to the cutaway cable housing that the loop comes thru on the backside?
