Has the AAD ever malfunctioned on you?

An AAD can be a life-saving tool and many skydivers use one.

But has one ever malfunctioned on you? Did it fire the reserve under conditions it shouldn't have? Did it not operate when it should have? Any other potential f-up on the part of the AAD?
