My nickname since high school was "Halls"
It rhymed with my last name a buddy seen on the side of a truck on the way to school and just stuck.
I have since then been known by Hallls Gaals.
Never ed really ed bothered me considering the nickname I've given other friends

Unfortunately no matter how you go about it I don't think it will remain anonymous no matter which route you take.
There is already a conflict because of this person slackness so emailing or writing up the person will not more then likely not cause anymore conflicts then that already are there.
I say bring this to the attention to someone higher up sooner then later.

Is there a way that you can "write" the person up on an official note to the boss that the said person is not up his ass?
In my field of work it's not official unless it is on a printed page.
