Unfortunately no matter how you go about it I don't think it will remain anonymous no matter which route you take. There is already a conflict because of this person slackness so emailing or writing up the person will not more then likely not cause anymore conflicts then that already are there. Is there a way that you can "write" the person up on an official note to the boss that the said person is not up his ass? In my field of work it's not official unless it is on a printed page.
