In the past, I've seen some problems and some causes.

Piece launches or formations under stress.
It's just like a lot of aspects of life, it's knowing when to let go. When you have grips and it isn't going well, let go. The wind can take a 100-lb person and create a lot of torque during a 4-way launch. It could be somebody hammering into a formation, or merely dragging it apart.

floating
If the plane starts to stall while you are floating, you'll feel your feet get light. Shortly, the plane will play "crack the whip" with you. If your arm is extended when that happens, it will be tough. You may choose to snug your arm in tight and try to hold on, but let go if it's not going to work out.

door frame
If you are on the tail side of the door, don't cut it so close that you whack yourself on the door frame.
Also, don't let others push you into the door because they "cut the corner" on their exit.
