Hit by Plane Wing

Aynyone know what happened here? Mechanical problem with plane? Idiot pilot? If you pause at a certain point at 1:13 you can see what looks like the wing clipping the guy in the back of the head. Based on camera activity afterwards it seems like this guy was partially decapitated.

Terrible.
