Description
After a malfunction, the tandem master decided to cutaway too low for the reserve to fully inflate. At the moment of impact the reserve had not yet fully inflated. The passenger struggled in hospital for 9 days before dying. The tandem master sustained injuries but will survive.

This is the first known tandem fatality in Mexico.
