| Description |
| Jumper was on his first AFF skydive and experienced a spiraling main canopy after opening. The student then rode this turning canopy to several hundred feet where he then cutaway but was at a low enough altitude that the reserve was unable to open prior to hitting the ground at a high rate of speed. |
