| Description |
| Jumper was following along a tandem that he brought to the DZ. In freefall the Tandem Instructor said he seemed disoriented and tracked off early. The jumper failed to deploy either canopy prior to impacting the ground. Medical issues are suspected to have caused the initial disorientation. |
