Air getting under student's goggles, probably making it harder to see, diminishing the value of the jump she paid for. Instructor might have done better to ensure the goggles were tight & properly placed, but it could be due to using Kroops, as they are notorious for having crappy elastics that wear out quickly.

Most TI's I know don't put a helmet on tandem students, the only ones I have ever seen actually put them on is at the tandem course, they put people in frap hats.

The contracts I signed with the Manufacturers,.. which falls under the FAR's. Are there no more contracts with the manufacturers or are we just not reviewing them after being signed?

Oy! The manufacturers of the... helmets? harnesses? I would like to know this, as I have gone on three tandem dives and got only a frap hat each time!
As far as the student not having an alti - that's also a big no-no! I did notice the left RSL line was out of its channel a bit - not quite where you want it.

I guess I'm left with no more than an "opinion" that if one feels the necessity to wear a brain bucket on their behalf; they should be inclined to do as much; if not more for the student.
And, we know of one such student that "might" have had a better outcome if more than just a frap hat.

That quote must be from before he backed the BOD when they deleted the requirement on Tandem Student to have a Altimeter, and before he started saying to many that the Alti on the T-I was "available" for the Student if need be.
