First part is about the Hawaii incident, second part about an incident in Switzerland. Translation:
A 52 year old skydiver fell to his death saturday afternoon in Sitten(canton Wallis), Switzerland. The police say that the man from the canton Waadt was doing a 4000m jump at the local aeroclub. For unknown reasons his main parachute did not open. The reserve parachute deployed automatically but could not inflate. So the man fell to his death. The government started an investigation on the exact cause of the incident.

Translation:

Preliminary report on the fatal accident that happened during a jump weekend of the Para Club Valais on Saturday 07. January 06 in Sion(Sitten).

Weather: No clouds, almost no wind with temperatures slightly below the freezing point.

Short info: Around 15:20 a licensed skydiver with 1300 jumps fell to his death on a field near the Sion airport.

After a 2-RW jump from 4000m/G the deceased reached for his BOC hand-deploy at 1500m/G, but couldn't get the pilot chute out of his BOC pocket. As a result he became unstable, without being able to get the pilot chute out of his BOC pocket. The rest of the jump is not documented and the witnesses could not give an accurate description.

The skydiver died because of an extremely hard impact. In the final position he was found under the fully opened reserve. The pod of the reserve was 15 meters above him in a tree.

None of the three handles (main, cut-away, reserve) has been used. After the accident all three handles were freely accessible and easily moveable.

The Cypres cut the reserve loop. The investigation will show why the reserve did not open early enough to save the jumper.

The other technical equipment (Cypres, Pro Track, etc.) is part of a judicial investigation. The results will be given to Swiss Skydive in due time.

Guy-Philippe Ayer, instructor and accident expert of Swiss Skydive has been comissioned with the investigation. The above text is part of his investigation.
